Please fill out the form below to register for childcare on October 14, 2011. This program is offered for CSCA children in Kindergarten through 5th grades. We will have games, crafts, a movie, snacks and fun learning to play soccer with the Girls Varsity Soccer Team! For lunch, pizza will be provided. This form and your attached payment will reserve your child's spot. The registration deadline is Friday, October 7th. Please sign up early, as space is limited. Also, we will not be able to accept unregistered children at the door. Please deliver this form to the Elementary Office or to Kelle Hunter in the Media Center.
All money raised will go to the Girls Varsity Soccer Program.
Child care will be at First Presbyterian Church on Royal Palm Blvd and not at the school.
