Matricultaion Fee (Not Refundable)

Preschool --------------- $200.00 ----- If Paid Before February 1, 2010 -- $150.00

Elementary School --- $600.00 ---- If Paid Before February 1, 2010 -- $485.00

Middle School --------- $700.00 ---- If Paid Before February 1, 2010 -- $575.00

High School ------------ $800.00 ---- If Paid Before February 1, 2010 -- $665.00

CSCA will bill me (us) in January 2010 for the lower matriculation fee. If I (we) do not re-entroll and pay the matriculation fee by January 31, 2010, the fee will be increased to the full amount. I (we) understand that the above fees, once paid, are not refundable.

Annual Plan (Full Tuition Due July 1, 2010; 4% Discount)

Semester Plan (Half Tuition Due July 1 & Dec. 1, 2010; 2% Discount on Both)

Monthly Plan (1/11 Tuition Due Monthly, July 1 - Dec 1, 2010 & Feb 1 - Jun 1, 2011)

I (we) understand that tuition is not refundable after classes start. I (we) are responsible for 100% of Annual Tuition regardless of which plan we choose or when we withdraw. Refer to the Schedule of Tuition & Fees on the CSCA website for details

I (we) authorize CSCA to release records and recommendations to post-secondary schools as needed, and release CSCA and its officers and assigns from any and all liability from providing records, recommendations, or evaluations on behalf of my (our) child

I (we) understand that keeping my account current will prevent the retention of records, including report cards and transcripts, and if my account becomes 60 days past due, I (we) will be asked to keep my (our) child home until the account is current
